 Health Insurance Plans
We bill for the insurance(s) with whom we are contracted for both Primary and Secondary Insurances. Specifically, we will submit billing directly to your insurance company. We ask that you provide us with all necessary information to accomplish this. Should you have secondary insurance, we will submit to the primary carrier, and after your primary carrier processes the claim, we will submit the balance to your secondary carrier. We participate in many different PPO and HMO plans and accept traditional indemnity insurance. If you are not sure about our provider status with your insurance company, please check with your insurance company or with our business office during normal office hours.

Sunrise Family Medicine is committed to providing the highest quality of
family medicine at a reasonable fee.We participate in many different PPO and HMO plans and we accept most
insurances.Upon visiting our office please remember the following important points with
regards to your insurance.
1. If you do not find your insurance carrier on our "Insurance List", which
you can locate on our website (www.sunrisefamilymed.com), please call the
phone number on the back of your insurance card and verify that we are a
Select Provider for your insurance.
2. At the time of service, you will need to bring a copy of ALL insurance
cards.
3. All other insurance carriers with whom we are not contracted with will
be billed as CASH.
4. All CoPays are due at the time of service.
5. We accept Visa, Mastercard, Debit Cards and Checks.
VISA AND MASTERCARD SYMBOLS ONLY (WE NEED TO ONLY PUT THE VISA AND
MASTERCARD SYMBOLS)
NO INTEREST PAYMENT PLANS ARE AVAILABLE FOR THOSE WHO DO NOT HAVE INSURANCE!
