Welcome to the Office

New patients are ALWAYS welcome to our practice. Sunrise Family Medicine believes in always caring and accepting any patient in need. When requesting an appointment, please inform the appointment secretary that you (or your family member) are new to our office. We prefer to schedule with the appropriate time in order to obtain a complete history and physical on your first visit.


What is "Walk-In Clinic Time"?

The purpose of the "Walk-In" is to expedite the providing of care for you or a sick family member who may have an acute or serious illness and to keep your waiting time to a minimum. The exam and treatment will focus on you or your family's immediate problem. Please understand that requests for other services during "Walk-In Clinic Time" will need to be provided during another visit.


Contacting the Physician During Business Hours

Many family problems may be easily handled over the phone. However, only through examining your family member can your Family Physician evaluate the problem. Our nursing staff will make every attempt to answer your questions, but we do understand that there are some cases that only the doctor will be able to handle. In these cases, we encourage you to make an appointment that day or later in the week, if the problem permits; but, if the problem is more urgent, the doctor will call you back as soon as he or she is free. For less urgent matters, please allow 24 hours for a return call.


Contacting the Physician After Hours

A Family Physician will be available for emergency calls after hours. Please limit your after-hours calls to those matters that you feel are absolutely necessary and "cannot wait" until regular business hours. Please have paper and pencil ready when you call to write down instructions. If indicated, take your family member's temperature prior to calling. Please have your pharmacy phone number available. Be prepared to answer questions about your family member's illness.


Office Visits

So that we may provide you with the best possible service, please be aware of and accept the following guidelines:

  • When calling for routine appointments, please call several weeks in advance to allow for convenient scheduling.
  • When making appointments for a sick family member, call early in the day and have available pertinent information (such as the family member's temperature). If your family member has a chronic condition or complicated medical history (such as physical, emotional or behavioral problems), please inform the appointment secretary so that we can schedule the appropriate amount of time.
  • If you feel your family member is contagious (chicken pox, measles, etc.), please notify us at the time of making the appointment. We will recommend that you enter from the back entrance.
  • When you arrive at the office, please sign in at the reception desk. On each visit, you will be provided with a consent form and update sheet that list the demographic information we have for you. Please review the information, make any changes, sign and return to our receptionist.
  • Write down your questions prior to coming to the appointment. Make sure your questions are answered and that you understand the answers.
  • If cancellation of an appointment is necessary, please call us as far in advance as possible.
